Instructions for using the VA 1010 form in Microsoft Word

First, you must determine where your Microsoft Word Template files are saved.  To do this, open Microsoft Word, click on the Tools menu, click on Options, and File Locations. You want the directory shown next to User Templates.

Download the file by right-clicking on the link (VA1010.dot) and selecting 'Save Target As...'.  Make sure the file name is VA1010.dot.  You may have to change the file extension from .doc to .dot.  If you don't do this, Word will not recognize it as a template. 

To use the template –

1.    Open Microsoft Word
2.    Click on File, New
3.    Select VA1010, then click OK
4.    The cursor is automatically located in the blank to the right of Study Name:
5.    Begin typing where the cursor is located
6.    Use the TAB key to move between fields.
7.    For fields with multi-line input (i.e. Address), use the ENTER key to
       start a new line and the TAB key to move to the next field.
8.    Please include the patient’s COUNTY, in field 4 (ADDRESS)
9.    Field #8 (Place of Birth) MUST BE FILLED IN or we cannot
       schedule the patient.
10.  Note that on Field #3 (Sex) and Field #9 (Marital Status), the TAB
        key will move sequentially through the choices.  Please place an ‘x’
        in only ONE box and press the TAB key until you get to the next
        field.
11.  After entering data in Field 10c (Telephone Number), pressing the
        TAB key will skip down to Field 15 (Social Security Number).  The
        information between fields 11 and the first field 15 (VA claim
        number) are not required.
12.  ALL highlighted fields MUST BE FILLED IN or we cannot schedule the patient.
        If you happen to have a form without highlighting, you must fill in the following fields :
        Study Name; Date; New/Return Patient;Veteran Status; Ethnicity/Race; Patient Name;
        Sex; Address (with County); Phone number; religion; date of birth; place of birth; marital
        status; emergency contact - name, relationship, address, phone; patient SSN (all 9 digits)
13.  If you want to save this as a document, you must use the File, Save As option. 
       
DO NOT save on top of the template file.
14.   The form can now be emailed to Yolanda Martinez 
        (martinezy@uthscsa.edu).  
        If you desire, you may also print it and then fax it to 567-4651.
