Checklist for your application to the MSCI Program:

1.      All originals of the items should be sent to the Registrars Office by: June 1st for the Fall semester; October 1st for the Spring Semester; and March 1st for the Summer Semester.

2.    Enter answers to all questions on the application and submit it

3.      Research Plan:  (Three to five pages; should describe Specific Aims, Background and Significance, Experimental Methods and Procedures, Long-Term Objectives).

4.      Attach a one-page description of your research and career goals and how this proposed research plan fits into your career objectives.

5.      Curriculum Vitae of applicant.

6.      Proposed mentor’s two-page NIH format Biosketch including Other Support.

7.      Attach a Copy of your Medical License or other professional accreditation.

8.    Health Insurance Coverage Information Form with a copy of insurance card (front & back)

9.    Immunization Records form

10.  Course Card

11.    Four letters of recommendation.  Address to Dr. Michael Lichtenstein and sent directly to the Registrars Office.

a.      Mentor:  documenting commitment to candidate’s career development

b.      Section Chief:  assuring 80% protected time for research.  This letter must specify how much time is to be spent on call and in clinic.

c.      Faculty Member

d.      Department Chair

12.    Request your transcripts.  Request that they be sent to the Registrars Office at the address below.

13.   Take the Test of English as a Foreign Language (TOEFL) if required.  Request that the scores be sent to the Registrars Office at the address below.

14.  Make a copy of all documents and send them to:
