WOC procedures and forms
To obtain a WOC (With Out Compensation) status within the VA Hospital, please print and complete each of the following forms and return them to Room A703 in the GCRC before any processing can be done.
OF 306 - Declaration for Federal Employment
OF 612 - Optional Application for Federal Employment
Nurses also must complete the
VAF 10-2850a Application for Nurses and Nurse Anesthetists and will need to provide the following -- Official transcript
- Reference from most recent employer
- Nursing license
- Evidence of current BCLS
If you are employed as any of the following occupations -
- Certified Respiratory Therapy Technician
- Registered Respiratory Therapist
- Licensed Physical Therapist
- Licensed Practical/Vocational Nurse
- Licensed Pharmacist
- Physician Assistant
- Expanded-Function Dental Auxiliary
- Occupational Therapist
you must fill out the VAF 10-2850c - Application for Associated Health Occupations
